Mga higala! Cagayan de Oro’s City Health Office (CHO) is now offering convenient online processing of MEDICAL CERTIFICATES to lessen physical visits and avoid crowding in their office!

Online Process Flow:
1. Online processing of requirements
2. IF approved, an online appointment and priority number will be given
3. During the scheduled time and date – present the original copies of the
requirements for assessment
Requirements:
1. Pertinent Personal data – Complete name, age, address, birthdate, blood type,
nationality, gender, civil status
2. Recent Photo (1×1 or 2×2)
3. Barangay certification that states the completion of quarantine or absence
of COVID19 symptoms for the past 14 days (subject for verification by the
CHO) – for travel purposes only
4. Recent Chest X-ray (w/in one month) preferably X-ray results with a photo of
the client
APOR (Authorized Personnel Outside of Residence):
1. Certificate of Employment
2. Company ID
3. Recent Chest X-ray (within one month) preferably X-ray results with a photo of the client
4. 1×1 recent photo (white background)
Non-LSI & OFWs:
1. Barangay Certification for Non-LSI
2. Recent Chest X-ray (within one month) preferably X-ray results with a photo of the client
3. 1×1 recent photo (white background)
