14 Oct You Can Now Get Your COVID-19 Vaccine Certificate @ CDO’s City Health Office
Mga higala! Kagay-anons who don’t have internet access can now get their COVID-19 Vaccine Certificate at the City Health Office (CHO) or the City Health Insurance Office (CHIO)!
ICYDK, this vaccine certificate is free of charge and can be availed after at least 48 hours since your full vaccination. Just bring your valid ID and your vaccination card.
BTW, the said certificate contains the following details:
- personal information of the vaccinee
- vaccine brand and lot number
- location and date of vaccinations
- QR code matching the data from the Vaccination Information Management System of LGUs
